Michelle Cohen, M.D. is a board certified internist who has been a partner at Medical Associates of Delray since 2008. Dr Cohen is originally from Johannesburg, South Africa. She relocated with her family to the United States in 1988. Dr. Cohen attended the University of Florida, where she graduated Phi Beta Kappa with a Bachelor of Science degree. Dr. Cohen subsequently attended the University of Miami School of Medicine in 1991 and went on to complete an internal medicine residency at Jackson Memorial Hospital in 1998. Dr Cohen has been practicing as an internist in the South Florida area since that time. Although she treats a wide range of acute and chronic conditions in adults of all ages, Dr Cohen has extensive experience in geriatric medicine.
Dr Cohen is married and has 3 children. She loves to travel. In her spare time, Dr Cohen is a mentor in a community mentor program.

Jose (Joe) L. Conde, M.D., is a physician and partner of Medical Associates of Delray, P.A. He joined his current practice in 2006. Dr. Conde holds double certifications by the American Board of Internal Medicine (ABIM) in both Internal and Geriatric medicine.
Dr. Conde Specializes in Adult and Geriatric Medicine. Patients range from 18 to 100+. Dr. Conde enjoys teaching clinical medicine. Since 2010, Dr. Conde has served asa Community Based Clinical Affiliate Assistant Professor of Integrated Medical Science Internal Medicine & Geriatric Division of Florida Atlantic University Charles E. Schmidt College of Medicine in Boca Raton.
In addition, he works for Hospice and Palliative care on the weekends treating patients in facilities and/or on their home.
Dr. Conde moved to Delray from Miami and attended Atlantic Community High School. Here, he excelled as a scholar athlete, achieving all-state football recognition while graduating at the top of his class.
Dr. Conde completed his medical residency at Saint Louis University where he served as Chief Resident of Internal Medicine, 2002-2005. He then completed a Geriatric Fellowship at University of Miami Miller School of Medicine in 2005-2006.
Dr. Conde is married with 3 children. In his spare time, he enjoys family time, coaching basketball, exercising, and traveling.

Dr. Silvia Coronel is a board certified internist who was raised not far from Medical Associates of Delray office and is happy to be providing primary care in her hometown. She began her professional education at the University of Florida in Gainesville, Florida where she earned her Bachelor of Science in Microbiology. She then completed a Masters degree in Biomedical Science at Tuft’s University in Boston, Massachusetts. She was accepted to Florida International University College of Medicine and earned her Doctor of Medicine in 2015.
Dr. Coronel matched into the University of Southern California Internal Medicine residency program where she trained both at Los Angeles County Hospital and Keck USC Hospital. After residency she worked as a hospitalist at Torrance Memorial Medical Center in Torrance, California, and then Bethesda East Hospital in Boynton Beach, Florida. She is married with 2 child, proudly bilingual in English and Spanish, and is accepting new patients

Jenna Freitag PA-C has been practicing in south Florida since 2013. She is originally from Marlton, NJ. She went to Florida Atlantic University for her undergrad and Keiser University for her Masters in Physician Assistant. She has practiced in family medicine, urgent care and internal medicine in her career as a PA.

Tammy has been a nurse practitioner since 2018. Prior to working in internal medicine she worked in pulmonology and hospital medicine.
She began her nursing career in 2006 after obtaining her Associates Degree from Corning College. She obtained her Bachelor’s degree in 2014 from Broward University and her Masters degree from Florida International University in 2017.
